General Information
Appointments
We see patients by appointment, unless there is a situation that requires emergent care. In this event, the physician will determine whether you need to go to the emergency room immediately or if we can accommodate you in the office the same day. Please give a 24 hour cancellation notice if you are not able to keep your scheduled appointment. Failure to do so will result in a $25 fee.
If you have a managed care or HMO plan, please acquire a referral authorization from your primary care physician for a new or return office visit. If an authorization is not on file at the time of the visit, we will have to reschedule your visit or collect payment in full for services rendered.

Payment Information
General
1) Fees for services rendered, along with unpaid deductibles, co-insurance and/or co-payments are due at the time of service. Checks, cash, Visa, MasterCard, American Express, and Discover are accepted.
2) Our business office will process insurance claims for reimbursement, as long as all information is provided at the time service.
3) Any charges not covered by the insurance company will be your responsibility. We ask that you familiarize yourself with your insurance benefits to avoid any unexpected non-covered charges.
Medicare
We accept Medicare. Our business office will submit claims to Medicare and to secondary or supplemental insurance. The coinsurance amount (which is 20% of the Medicare-Approved Charge), any unmet deductible, and all non-covered charges are due at the time of service, if there is no secondary or supplemental insurance.
Medicare will pay for a hearing test to determine whether a hearing aid(s) is needed. However, Medicare will not cover the cost of a hearing aid(s). Should you decide that a hearing aid(s) is right for you, a deposit will be collected at the time of order and the balance will be due upon delivery of the hearing aid(s).
Surgery, Hospital and In-Office Special Testing
Our business office will verify insurance coverage and submit claims to insurance carriers for these special services. Non-covered services, co-payments and/or deductibles will be collected in advance.
